چکیده

Methods In 42 HLHS patients (6.0 ±2.3 yrs) and 14 healthy volunteers (6.8 ±3.9 yrs), MRI first-pass perfusion (0.03 mmol/kg Gd-DTPA; TR/TE/a=2.6/1.1/20°) and late gadolinium enhancement (LGE) imaging was performed using a 3 Tesla scanner (Philips Achieva). Quantitative myocardial blood flow at rest and stress (Adenosin 140 mcg/kg/min) was calculated in 4 anatomical RV segments per slice using a model independent deconvolution. A total of 672 segments were analysed. CMR results were compared to conventional x-ray guided coronary angiography in all HLHS pts.

Imaging Maladaptive Aortic Properties in Children After Palliation of Hypoplastic Left Heart Syndrome Assessed by Cardiovascular Magnetic Resonance Imaging

Background—The status of the reconstructed aorta in hypoplastic left heart syndrome is considered an important determinant of long-term prognosis. Therefore, we assessed the anatomy, elastic properties, and viability of the aorta and right ventricular function in patients with hypoplastic left heart syndrome by cardiovascular magnetic resonance imaging. Methods and Results—Cardiovascular magnet...

Surgical palliation strategy does not affect interstage ventricular dysfunction or atrioventricular valve regurgitation in children with hypoplastic left heart syndrome and variants.

BACKGROUND All 3 palliation strategies, Norwood, Sano, and Hybrid, currently used for hypoplastic left heart syndrome pose a risk of myocardial injury at different times and through different mechanisms. We sought to compare these strategies to understand longitudinal differences in interstage ventricular dysfunction and their subsequent impact on transplant-free survival and atrioventricular v...

Global improvement in right ventricular function after Stage II Norwood operation in children with hypoplastic left heart assessed by serial MRI

Introduction Hypoplastic left heart syndrome (HLHS) describes a spectrum of left heart hypoplasias, which leave the left ventricle unable to support the systemic circulation. Staged palliation consists of aortic reconstruction and systemic to pulmonary artery shunt in the neonatal period (stage I), followed at intervals by superior (Stage II) and then total cavopulmonary connection (Stage III)....

The myocardial and coronary histopathology and pathogenesis of hypoplastic left heart syndrome.

Hypoplastic left heart syndrome has the greatest mortality rate among all CHDs and without palliation is uniformly fatal. Despite noble efforts, the aetiology of this syndrome is unknown and a cure remains elusive. The genetic and anatomic heterogeneity of hypoplastic left heart syndrome supports a rethinking of old hypotheses and warrants further investigation into the histological and vascula...

Norwood Procedure for Palliation of Hypoplastic Left Heart Syndrome: Right Ventricle to Pulmonary Artery Conduit vs Modified Blalock-Taussig Shunt.

Patients with hypoplastic left heart syndrome undergo a series of operations to separate the pulmonary and systemic circulations. The first of at least 3 operations occurs in the newborn period, with a stage I palliation. The goal of stage I palliation is to provide pulmonary blood flow and create an unobstructed systemic outflow tract. Advances in surgical techniques and intraoperative and pos...
